OBJECTIVES:  What will the student accomplish?
The curriculum is designed to meet the health and fitness needs of the student and provide the opportunity for participation in various athletic endeavors (such as sports and fitness programs) and leisure activities that involve exercising and conditioning the body. 
Health includes subjects such as nutrition, diseases, and substance abuse.  The health portion of the program will be provided to the student each month in the form of a magazine, Choices.  Each issue is designed to meet current National Health Education Standards.  Magazines will be handed out by the contract teacher during Advisory four times per semester.  If absent, students are responsible for picking up the current magazine from the contract teacher
upon their return to school.

Goals*:  Meet the National Health Education Standards in the Health  learning areas of Health Promotion/Disease Prevention, Using Health Resources, Practicing Health-enhancing Behaviors and Reducing Health Risks, Analyzing the Influence of Culture on Health, Using Interpersonal Skills to Enhance Health, Using Goal Setting and Decision Making for Health and Advocating for Personal, Family and Community Health.

ACTIVITIES*:  State the planned activities to accomplish the Health requirements:

  1. Read and complete 4 summaries of articles from each “Choices” magazine. Summaries will be from the main articles, not the short clips in the beginning and end of the magazine.  Summaries will include at least 4 major points of each article as well as a reflection of their thoughts about the article in paragraph form. The student is required to read the entire magazine each month and submit the summaries in the correct format: 1-2 pages, double-spaced, typed, 10-12 font. Total number of summaries: 16
  2. Complete one of the following:
    1.  Write a 4-page typed research paper, 10-12 font, on a health topic of your choice.  The paper should include a bibliography.
    2. Attend 3 hours of community health presentations and submit a log of those hours, verified by a parent or guardian.
    3. Watch 3 one-hour programs available on TV, DVD, video or online on various health or fitness topics and submit a log of those hours and programs, along with a short paragraph of what the program was about and health topics covered, verified by a parent or guardian.
    4. Do a combination of b and c.  Submit a log of hours, verified by a parent or guardian.

  1. Take a Classroom Based Assessment (CBA) at the end of the semester during Advisory (to be announced).


Note:  completion of Health requirements equals about 15 equivalent class hours per semester.  A total of 30 hours of Health is required (2 semesters).









Fitness The primary goal of a fitness curriculum is to insure that the student gets a necessary amount of exercise and acquires appreciation and knowledge of an activity or activities to enable him/her to continue participation beyond the high school years.  The activities need to include the development of strength, flexibility, and endurance.

  1. Complete the attached PE Log of activities and hours including a journal entry every 2 weeks on insights and goals attained.

EVALUATION:  How will this contract be evaluated?
            "Pass"              Completion of contract time of 60 hours in planned physical activities, 15 hours health, research paper and a CBA.

            "B"       Completion of contract time of 70 hours in planned physical activities, 15 hours health, research paper and a CBA.


            "A"       Completion of contract time of 80 hours in planned physical activities, 15 hours health, research paper and a CBA.


            "F"       No credit given.  Work not completed.

Demonstratable contract hours should be in activities which are clearly geared toward physical activity and fitness.  Examples of hours are those spent in activities such as sports, running, walking, weight-training, cycling, yard work, swimming, medium to heavy labor, exercising at a gym, etc.  Activities less than 15 minutes in duration will not be counted.

We have planned my goals and activities for my semester of physical education and accept this contract as it is written.  It is my responsibility to complete this contract in a timely manner and meet with my supervising teacher as needed to accomplish my goals.  The hours will be verified by the parent as true and accurate.


*** Note*** Documentation of hours will be required to be submitted during each Advisory Period.  Failure to provide documentation for 2 Advisory Periods will result in a deduction of 1 letter grade.  In the case of a Pass/Fail option, the student will revert to a letter grade which will be lowered.
